No es infrecuente que algunos pacientes con enfermedades avanzadas expresen cierto deseo de morir como consecuencia del sufrimiento multidimensional (físico, psicológico, emocional, social, espiritual-existencial) que puede darse en estas circunstancias de final de la vida. Asimismo, en este contexto, algunos pacientes también pueden experimentar cierta pérdida de sentido de dignidad o de la identidad, entendida como cualidad intrínseca e irrenunciable del ser humano. A la luz de los estudios que han explorado la experiencia de la enfermedad en pacientes con enfermedades avanzadas puede observarse que, a menudo, el sufrimiento que experimentan, sus angustias y temores, no son la descripción de su situación clínica actual, antes bien, tienen que ver con la anticipación de una situación futura (dolorosa, incierta, desconocida) que no serán capaces de manejar.
La vivencia de la temporalidad, por tanto, es un aspecto fundamental para com-prender la experiencia de la enfermedad de muchos pacientes que viven con una conciencia extrema del límite temporal de la vida. En este análisis se presentarán algunas ideas filosóficas –principalmente desde la perspectiva de la fenomenología de la corporalidad– para aproximarnos a la cuestión de la anticipación del dolor, y se pondrán en relación con la experiencia vivida, tal y como está recogida en algunos estudios cualitativos sobre el deseo de adelantar la muerte, la pérdida de percepción de dignidad y de autonomía en pacientes al final de la vida.
