Abstract

Background: Myelophthisis means invasion of the bone marrow by foreign neoplasms. The importance of demonstrating this entity lies in the poor short-term prognosis. In our environment there is little clinical and histopathological information about this entity. Aim: To know the hematopoietic morphological and microenvironment findings in bone marrow biopsies with metastases. Materials and methods: Retrospective, cross-sectional, descriptive, observational and open study in patients with a diagnosis of myelophthisis from January 1, 2017, to December 31, 2021 in bone marrow biopsies from the Department of Pathological Anatomy of the “Antonio Fraga Mouret” Specialty Hospital Medical Center National “La Raza”. Results: 143 bone marrow biopsies were reviewed, 39 demonstrated metastasis (27.7%). The absolute frequency of myelophthisis was 0.669%. The most frequent metastases: breast carcinoma (23%), prostate (20%) and Neuroblastomas (15%). The most common current condition: anemia (50%). The hematopoietic morphological findings were a decrease in megakaryocytes, poor granulocyte maturation and erythroid hyperplasia, and non-hematopoietic fibrosis, osteoblastic and osteoclastic reaction. Conclusion: The assessment of metastasis in bone marrow biopsy continues to be key for diagnostic confirmation; our study demonstrates the morphological findings evidenced.
